Amelia Lavenski, a 4-year-old girl from Anne Arundel County, Maryland, has been dealing with epilepsy since she was 15 months old. Epilepsy is a neurological condition that causes seizures. Amelia also has a rare gene mutation called mitochondrial disorder, which leads to prolonged seizures. Previously, Amelia would experience one or two seizures every year that required hospitalization. However, in August 2023, she had an uncontrollable seizure that led to significant changes in her life. She was rushed to the emergency room and admitted to the Johns Hopkins Children's Center Pediatric Intensive Care Unit (PICU). Despite efforts to stop the seizure with medication, it continued for a week. To prevent further complications, Amelia was intubated and monitored using an electroencephalogram (EEG), a test that detects brain activity. Eventually, Amelia's condition improved and she woke up from the prolonged seizure after her medication was adjusted and the ketogenic diet was implemented. The ketogenic diet is an old but effective treatment for epilepsy. It involves consuming high levels of fat and very few carbohydrates to help control seizures. Unfortunately, as a result of her prolonged seizure, inflammation occurred in Amelia's brain, causing severe complications. She lost the ability to walk, move one side of her body, sit or hold up her head on her own. After spending two weeks in the PICU at Johns Hopkins Children's Center, Amelia was moved to a step-down unit where her care team devised a treatment plan going forward. This plan included continuing with medication and maintaining the ketogenic diet for now. The ketogenic diet has proven beneficial for many individuals with epilepsy; however, it is not effective for everyone. In Amelia's case though, the implementation of this particular diet proved lifesaving. After three weeks at Johns Hopkins Children's Center,PICU ,Amelia was discharged home and then spent five additional weeks at Kennedy Krieger Institute for inpatient rehabilitation. Now back home, Amelia has mostly remained seizure free. She initially continued with the ketogenic diet and has only recently transitioned back to a regular diet. Amelia continues to undergo at-home therapy and outpatient therapy to relearn how to walk and regain other social and motor skills. Her doctors are hopeful that she will continue to make progress as her recovery continues. Amelia's story showcases the importance of personalized medical care and treatment options like the ketogenic diet in managing epilepsy. As medical professionals, it is vital to prioritize individualized plans that consider each patient's unique needs. By providing this specialized care and utilizing treatments like the ketogenic diet, healthcare providers can help patients like Amelia improve their quality of life and achieve positive outcomes.
